About Us
My name is Nathan Yoder and I have been a licensed physical therapist with over 25 years of experience having worked in various clinical settings including outpatient, acute hospital rehab, skilled nursing facilities, intensive care units and assisted living facilities.
I started At Home Outpatient Physical Therapy LLC with the goal of helping seniors to remain at home as they age instead of being placed in a nursing home type of facility by helping them improve their function, safety and independence in their home environment.

Initial Assessment
Initial Assessment is designed to provide a comprehensive evaluation of your physical condition and develop a tailored treatment plan. During this session, the physical therapist will conduct a thorough review of your medical history, perform a detailed physical examination, and assess your functional movement patterns. We will discuss your specific concerns and goals to ensure our approach aligns with your needs. Initial evaluation typically lasts about 60 minutes.
Treatment Visits
Our Regular Treatment Visits are designed to provide ongoing care and support as you progress through your recovery journey. Each session is tailored to address your evolving needs and goals, building on the initial assessment to ensure continuous improvement. The physical therapist will utilize a variety of techniques and therapies, including gait and balance training, targeted exercises, and education on injury prevention. Sessions typically last 45-60 minutes.
Home Exercise Programs
We will equip you with self-treatment techniques and exercises to continue your progress at home, helping you stay engaged and proactive in your recovery. Our goal is to empower you with the tools and knowledge needed for long-term success and to support you every step of the way.

Do I have to be homebound?
No you do not need to be homebound to receive outpatient physical therapy services in your home. -
Are your services covered by insurance?
Yes, we are a proud provider of Medicare part B services.
Currently we do not accept any Medicare Advantage plans.

4. When can I begin physical therapy?
Our services can be initiated at any time with a referral from
your healthcare provider.
